For reference, they'll send it shortly after your service is actually terminated (within a day or so), which is at the end of your current service month. This is unless you request an 'immediate cancellation', for which your service will be terminated immediately and you'll receive a credit for the unused time left in the service month (if it's already been paid for).
So you're aware in advance, this is the sheet that comes with the return kit, showing what you'll need to return and how to do so.
