where is my Rebate?

When i signed up I was offered a $100 rebate from the Hughesnet salesperson. I have never received the rebate. Im not sure if there is something I need to do in order to secure this rebate as that was unclear by the salesperson. could you please look into my account and give me any and all instructions I need to follow in order to receive this? 

My modem SN is B3 0010973107 A9

My start date was May 21
