What equipment needs to be returned when closing account?

We are moving and will have other options for ISPs, so will be closing our account. What equipment needs to be returned? I know about the modem and radio from the dish, but saw another post that mentioned a power pack? Not sure what that is and where to find it?

The power pack mentioned is the power supply for the modem.  

The following is a PDF that shows what needs to be returned, as well as how to remove the radio from the dish.  A sheet of this will also be included in the return kit.
